Human accident insurance

Claim documents includes:

1. Completed claim form

2. Certificate of Insurance (or copied list of Insured Persons)

3. Accident report with confirmation of the workplace manager, the authority or the police at the place of accident (in case of accident).

4. Records of treatment procedure: hospital discharge note, treatment record, surgical certificate, prescription…

5. Declaration of Death and the legal confirmation of the beneficiary or beneficiaries (in case of Insured’s death).

You can submit hard copy documents to one of the following addresses:

The claim processing time is within 15 business days from the date we receive all complete and valid documents, and is subject to the terms of your insurance policy.
If the processing time has been exceeded, please contact our Hotline at 1800 1118 and provide your claim number or policy number for assistance.
